Severe strain?

My abdomen on the lower right and middle right above and directly lower than my navel
and my back is hurting too.
I live beside my grandmother and she says that it is menstural cramps or a ulcerand not to verbs about it but this is approaching a tearing and knawing stomach-ache. that is made worse by moving walking lifting my principal up etc.

Can someone help me?
