This needs to be checked out right now. It sounds exactly like the problem that my friend's diabetic mom have. The rash turned into a sore that didn't treat and went gangrenous, and eventually she have to have her leg amputated.

I don't parsimonious to scare you, but a diabetic should know that correct foot care is critical to maintain your health. Please don't EVER consent to a foot problem go untreated for this long again.
