Why surgery? I lived beside someone with ingrown toenails and intellectual to treat them. First, after a bath or soak, you report the top of the nail until it's shrill - make sure you do the sides and don't walk too thin - afterwards you snip a small "V" in the center or for a while towards the side that's worse - on the corners where it's digging in stuff tiny pieces of cotton soaked in an antiseptic. Don't trim the corners shorter than the middle. Or you can enjoy surgery. Not really an answer to your question, eh?
