Do you think tiered co-pays are positive or refusal for their patients?

As someone who works contained by a pharmacy I see alot of patients that are perscribed one thing and next we have to phone and have the dr variation it becuase it is not perferred or because the copay is too high for the long-suffering to pay. I see it as a refusal for the patient as they arent getting what the dr orginially perscribed, the pharmacy spends lots of time on the phone trying to find out what the insurance company will cover cheaper in equal drug class instead of the original calligraphy, then we christen the dr and they spend time on what to change it to
The lone benefit i see is for the insurance company they are too cheap to pay for what some associates need or they hold little agreements for perferred drug companies...make it a generic copay or a brand copay!
