I agree that he should see an orthopedic doctor ASAP because he has shoulder instability that sounds pretty significant. Physical psychiatric help may be ordered as a conservative treatment and it may work but if it doesn't then surgery may be required. If the PT does work consequently he has to maintain up with the exercises indefinitely because PT will not cure the problem, it singular shows him how to manage the problem for the rest of his duration.

There is defacement or a problem with his shoulder socket. A woman at work have similar trouble with both shoulders for which she eventually needed surgery. She tried Physical Therapy, but within was minimal advance.When it is back within the "socket", he should at least permit it rest a few days so some of the injured tissues might start to heal. Using a sling would be a virtuous idea, so that it can really rest.
Definitely plan to see a doctor in the order of it.

Two words - Orthopedic Surgeon. A know someone who had equal problem. They had a pin put in the shoulder around 25 years ago, and no more problem.
