How are "pre-existing" conditions determined in healthcare coverage?

Since people recurrently switch healthcare plans yearly depending on time adjustments or profession changes, what's to determine if an ailment be "pre-existing"? Say I have cancer.. and don't seize diagnosed.. then switch job and it's diagnosed... would someone say that's "pre-existing"?. What are the details for this nebulous occupancy?
