I am wondering the difference between hydrocodone and codeine. I had a severe toothache and was prescribed Tylenol 3 300mg Tylenol and 30mg of what? Codeine? I didnt suggest you could be prescribed 30mg of codeine in one pill, I thought I had asked a doctor formerly but the answer was 10mg of hydrocodone was the maximum. Is codeine a weaker form of hydrocodone or are they impossible to tell apart thing with different chemical structures? If so, what mg of codeine would my tylenol 3 be and is it possible to know what that would equal contained by hydrocodone?

There are a couple of difference between these 2 molecules.

1) Potency: Codeine is a weak agonist. (It activates the opiate receptors weakly.) It is 20 times weaker than Morphine. Hydrocodone is a moderate strength agonist. It is 5 times more potent than codeine, or 4 times weaker than Morphine.

2) Efficacy: Given that Codeine solely binds weakly to the opiate receptors, it is not very efficacious as a pain reliever. Hydrocodone is far more efficacious.

3) Chemical Structure: You an give somebody a lift a look at both their chemical structures on wiki. There are 2 differences between the Codeine and Hydrocodone molecules.

(Structurally, Hydrocodone is 7,8-dihydrocodein-6-one. It is a codeine molecule with 2 modifications. If you have studied chemistry previously, the 7,8 double bond on the cyclohexene ring is first saturated yielding dihydrocodeine. Next, the 6-position is modified to concede a ketone, forming dihydrocodeinone or hydrocodone.)

Given that their potency varies, the dosage can be calculated like this:

200mg Codeine (oral) = 40mg Hydrocodone (oral) = 30mg Morphine (oral) = 10 mg Morphine (IV/IM)
