I get attacked...?

You really need to go to your doctor or to the ER. Without knowing your age and medical history and examining you, it is unyielding to say. There are a few different things that come to mind. The first thing is are you have problems with your heart. You will need an EKG and cardiac enzymes. I would take this checked out sooner rather than later. Take some aspirin very soon just to be safe. The second piece it can be is from your lungs. Do you have pneumonia or costochrondritis? Unsure. Take some Ibuprofen. If its costochrondritis, this can help. Is it from your stomach or pancreas? Could be next to the pain shooting into your back but unlikely. A simple blood testing will tell. And lastly, is it musculoskeletal? Until all of the above is ruled out, next only then you can enunciate it is. But either way, turn and make sure its not your heart.

The area you're describing have pain in could glibly be your heart. I'd go to a doctor. Even if it's your lung and not your heart, a doctor will be able to rule out more serious problems such as aortic spout regurgitation or stenosis and/or congestive heart failure.
