Medicare is federally funded insurance for citizens over the age of 65, or disabled. It is not based on income. Well, okay, in 2007, your Part B (doctor bill) coverage premiums are base on income.
Medicaid is state-funded, and thus the qualifications ebb and flow from state to state. If you have no income, you would imagined be covered. Google "medicaid" and your state name.
I'm not sure what an HRA is, unless you are conversation about a Health Reimbursement Arrangement. This is the side you can get if you hold a HDHP and do not qualify for a HSA.
How many hours do you sleep a year?
If you are unemployed, own a very low income and own no assets you can apply for Medicaid Managed Care.
