Surgeon desires money upfront because of diffficulty w/ Medicare?

My mother-in-law needs breast fall surgery. The Dr. Wants his fee ($6000) salaried before he does the procedure. He say that is because he have trouble collecting from medicare.

My mother-in-law says that she call medicare and got approval for the surgery.

My put somebody through the mill is how do I make sure the Surgeon make an effort to collect from Medicare. (it's my $6000)

Get a pre-certification communication from insurance company stating they are agreeing to pay "X" amount for this procedure. We hold to do this alot with expensive radiology procedures where on earth I work.
